This is a complex side quest with multiple routes, in which you'll either side with Cachino (the “good” option and better for Mr. House), or Big Sal and Nero (the “evil” option and worse for Mr. House). Either way, you can complete the Mr. House main story route without significant trouble, so it's okay to pick the Big Sal and Nero route if that's your bag.

How Little We Know Side Quest Walkthrough

Whatever route you decided to take in How Little We Know, the quest will end when you report the results back to Mr. House. However, if you sided with Big Sal and Nero, Mr. House informs you that he will have to divert his Securitron resources to defending against an attack from the Omertas during the final battle.
Either way, you'll receive your next assignment to investigate the Brotherhood of Steel in The House Always Wins V.
This quest is received from Mr. House immediately after finishing The House Always Wins III and accepting the next assignment.
Siding with Cachino by selling him back his Journal during the How Little We Know side quest will ensure that the Omertas do not attack The Strip during the final battle. This choice will also improve your reputation with The Strip.
On the other hand, siding with Big Sal and Nero will cause the Omertas to attack during the final battle regardless of your subsequent actions, and will lower your reputation with The Strip.
There are no rewards for completing this quest. All rewards are gained as part of the How Little We Know side quest.
