When your character wakes up after the intro, Doc Mitchell will greet you and help you create your character. In this section you can choose your character's name and appearance.
Note that you cannot change your appearance after unless you have the Old World Blues DLC.
After choosing the appearance of your character, Doc Mitchell will ask you to use the nearby Vit-o-matic Vigor Tester. This machine will allow you to choose the starting stats of your character.
When building your character, it is better to focus on select stats than making a jack-of-all-trades character. Despite feeling counter-intuitive, removing points from unnecessary S.P.EC.I.A.L. stats depending on your build is imperative.
For example, if you want to focus on a Two-Handed Melee build, increasing Strength and Endurance to 8 and removing 2-3 points from Perception is a good start.
After choosing your character's stats, Doc Mitchell will ask you to sit down on the couch. After which, he will ask you a series of personal questions and conduct some tests. These tests will be used to determine skills that you should start with. These can be increased further as you level up in the game.
You don't have to worry about the answers. At the end of the tests, you will be able to choose the skills you want.
After choosing skills, you'll be give the option to choose up to 2 traits. Traits provide a trade off of effects by providing you a bonus in one aspect while weakening another.
Note that you cannot change your traits after unless you have the Old World Blues DLC.
Once you are done creating your character, follow Doc Mitchell to the door. He will provide you a lot of items and your next possible quests.
