Speak With Caesar About His Illness

Converse with Caesar

During your conversation with Caesar about his illness, he will assign the task of curing him to you.

Treat Caesar's Illness

Fix the Auto-Doc

Steps to Fix Auto-Doc

Head to Vault 34. Once inside, look for the stairs and go two floors down to find the clinic.

Find the Auto-Doc and interact with it to get the parts you need.

Return to Caesar and activate the Auto Doc. Choose to either treat or kill Caesar.

Perform the Surgery

Steps to Perform the Surgery

Head to the New Vegas Medical Clinic.

Purchase a Doctor's Bag and Surgical Tools from Doctor Usanagi.

Return to Caesar and perform the surgery. You will need at least 75 Medicine or you can leave it to luck.

Have Arcade Gannon as a Companion

Fallout New Vegas - Arcade Gannon

If you have Arcade Gannon as a companion, you can skip this quest altogether by selling him as a slave to the Legion. You will get negative karma for doing so, and Arcade will no longer be available as a companion for the rest of the game.

Speak With Caesar or Lucius

Fallout New Vegas - Convince Lucius

If you successfully treat Caesar, he will get up from the bed and provide your next task.

If you kill Caesar at any point during the quest, whether intentionally or accidentally, you will be confronted by Lucius, blaming you for Caesar's death. If your Legion reputation, Medicine Skill, or Speech Skill is high enough, you can convince Lucius that there was nothing else you could have done, and you will progress to the next Legion quest. Otherwise, the Legion questline will fail and end, and the Legion soldiers will become hostile.

Treat Caesar

If you're planning on siding with the Legion during the final battle, treat Caesar during this quest to skip the extra step of convincing Lucius that you did what you could. Other than that, however, whether Caesar lives or dies is ultimately inconsequential.
