Aim at Mister Peeks on Crane
Hold your breath while aiming through a sniper scope at Mister Peeks, who is located in the middle of the crane. He will then jumpscare you, before giving you the reward rift with the Tombstone Perk inside.

Perk-a-Cola Machines
Tombstone Soda Cans can be purchased from Perk-a-Cola Machines for 2000 Essence. These are marked on your Tac-Map called Tombstone Soda, with an icon of a skull on it. Take note the locations for these are random and will change each game.
Completing Contracts
Completing a contract can give you a Tombstone Soda Can or a Tombstone Schematic as a reward from the reward rift. The rewards for these are random, so you're not always guaranteed that you will get a can or a schematic each time you finish a contract.
Crafting Schematics
You can craft a Tombstone Soda Can if you have its crafting schematics. Go to your loadout menu and hover to your backpack and select Add Item, then go to Schematic Crafting to find the Tombstone Soda Can under Perk-a-Colas.
Do note that there is a cooldown upon crafting, however the upside for this is that they are free to craft and have no additional cost.
How to Get All Crafting Schematics
Find Them In Loot Caches
The Tombstone can be found randomly inside Loot Caches. It is extremely rare to find, and it can be difficult to locate one using this method. However, you will be able to save 2,000 Essence compared to buying it from the Perk Machine.

Consume a Tombstone Soda Perk
The Tombstone perk needs to be activated first by consuming a Tombstone Soda Can. You'll know that the perk is active once you see the RIP icon on the bottom of your screen.

Head Over to Previous Death Location
Upon death while the Tombstone perk is active, load up into the next game and go to the location of where you previously died. You'll find a Tombstone on the ground and you can reclaim your gear by approaching and interacting with it.
If you forgot the exact location of where you died, you can look for the Tombstone Stash icon on your Tac-Map.
You Can Only Reclaim Items in Rucksack
Unforunately, you can only retrieve gear and items that are in your Rucksack and not the ones that you have equipped before dying.
