Modern Warfare 3 - Enemy Kills Using a Silenced Gun Without Taking Damage

Clean Kills are earned by scoring Operator Kills using guns without taking damage. You can still get Clean Kills even if you're affected by Tactical Equipment such as Stun Grenades or Flash Grenades, as long as you stay at full health and don't sustain damage.
