Head to the Sewers
Head to the sewers. For this quest, use the Mist Aken entrance to the sewers. You can either teleport there or run to the entrance behind one of the buildings in the slums.
Track Down the Perpetrator
Follow the quest marker and press onward into the sewers. This quest will open up a previously locked tunnel in the sewers.
Continue to follow the quest marker until you reach a group of Filthoids. Defeat them.
Keep Moving
Go left to the Wastewater Treatment Area Avian Tower and activated gate to arrive at the Wastewater Treatment Area.
Talk to the Panicked Victim
Go to the center of the room and talk to the cowering victim. He will tell you about the monster in the sewers and the other civilians who are trapped up ahead.
Unlock the Gate
Clear the drag puzzle to turn the valve on the wastewater treatment system and open the gate. However, instead of unlocking the gate, the valve will unlock the pipes, releasing a flood of Filthoids.
Defend the Civilian
Face off against the oncoming Filthoids and defend the civilian.
Stick close to the victim and don't let the Filthoids attack him.
Unlock the Gate
Once you have cleared the Filthoids, go to the valve next to the gate and unlock it.
Head Deeper Down the Sewers
Follow the quest marker to a flooded section in the sewers. As soon as you enter the new area, you'll notice a civilian trapped behind spinning columns with spikes.
Defeat the Filthoids
There will be 7 Filthoids in the area. Defeat them to eventually disable the device trapping the civilian.
Reactivate the Control Panel and Rescue the Civilian
This part of the quest will play out similarly to the Dead or Alive quest in which you have to defend the drainage mechanism to escape Lonza Fortress.
Turn the First Valve
Run to the valve closest to the trap and solve a drag puzzle to turn it.
When the first valve has been turned, more Filthoids will arrive. Keep fighting them while the second valve is being unlocked.
You can monitor the padlock icons at the top of the screen to see the unlock progress.
Turn the Second Valve
Once the valve has been unlocked, follow the quest marker down the stairs to another pipe.
This time, you don't need to clear a puzzle. Simple hold to turn the valve.
Expect another wave of Filthoids while the third valve is being unlocked.
Turn the Third Valve
Once the third valve is unlocked, jump down from the area where the second valve is to reach the other end of the flooded area.
Hold the button to spin the valve, disable the trap, and activate the drainage system. The civilian will run off as soon as the trap has been disabled.
After that, a short cutscene will begin. The rest of the Escort will take the civilians back to the surface while the rest of the group will decide to hunt down the perpetrator.
Drain the Wastewater
The end of the cutscene will take you to a different area in the sewers, back to the entrance to the flooded section. Follow the quest marker to another valve nearby. Simply interact with the valve to turn on the drainage system.
There will be Filthoids in the area, but you can choose to defeat them or simply move on.
Continue to Pursue the Perpetrator
The quest marker will take you deeper down to the Wastewater Treatment Depths. Keep following it until you reach the very bottom, where you will meet the monster that attacked you.

The Perpetrator, Subject 47, is a fairly challenging story boss in the Shadows of Dread subquest. It primarily hits you at close range and has a high attack speed. As the battle goes on the Perpetrator will begin to become more aggressive, but the battle will end when you take out half of its HP.
It is generally recommended to fight it using ranged weapons or characters with ranged Combat or Ultimate Skills. This is because the Perpetrator dashes away at frequent intervals.

Leave the Sewers
After the cutscene, follow the quest marker to the sewer exit. The group will find several soldiers face down on the floor.
Shadows of Dread Bugs
Wastewater Treatment Area Softlock
Many players have reported running into a softlock after successfully defending the civilian. When players solve the drag puzzle to unlock the gate, the gate will fail to open, preventing them from progressing in the quest.
Others have also reported sudden unexplained internet connectivity issues during the same part of the quest. As of now, there are no known fixes.

Perpetrator Boss
The Perpetrator of the Charon Hunt incidents, Subject 47, is a wolflike creature with quick movements and powerful close range attacks. Reduce its HP to half in order to win the battle.
