Wither Sabbat can kick you with its hind legs if you get too close. Either execute Perfect Dodge or just keep your distance.
Wither Sabbat charges at anything and everything around it, all the while in a headbutt position. Get out of the way or keep your distance.
It also stands on its hind legs to crash down with a headbutt. This attack has a long windup, so you'll be able to prepare yourself.
Its weak point is its back shell. Attack it while it is on all fours.

Get out of harm's way from its headbutts by performing a Perfect Dodge. Alternatively, you can keep your distance and use Psychokinesis to deal damage.
To maximize damage output, use Follow-Up psychokinetic attacks. You will be dealing a lot more damage this way in a short amount of time.
