The motion will determine the pose your character will make when initiating Path Actions. You may choose from three options that are exclusive to the physique you chose.

You are given six voice options in total, with three masculine voices and three feminine voices. You may play samples of each voice before you choose.
You can choose any voice regardless of your character's physique.

You may choose a skill that will help you in battle. Your character will keep this skill regardless of the Job you choose for them.

Once you choose a favorite dish, you will be prompted to name it. You will receive your chosen favorite dish at the start of the game.

A mushroom that can be maintained to yield a harvest of mushrooms at a later date. |
The belongings you choose determine which items you start the game with. This allows you to use their benefits earlier without having to find them while playing.
You can obtain the belongings you did not choose as you play through the game. This means that you will eventually be able to receive the benefits of each item eventually.

After you are done with customization, you can then set a name for your character, who will be the protagonist of the story of Octopath 0.

You can only change your Protagonist's features by unlocking the Salon at the beginning of Chapter 3 of the Kindlers of Flame Story.
Do note that also need to invite an NPC that can change certain aspects of your character, like your Favorite Dish or Name, but hair can always be changed at the Salon.
