First Phase

Shoot the Glowing Spores

Shoot the Glowing Spore

In the early stages of the fight, the glowing spore on Patient 13's body is its primary weak spot. You'll see several spores on its body, but the one that is highlighted in orange is the spore that you should be shooting at.

After dealing enough damage, a different spore on its neck and shoulders will start glowing so keep an eye out on which one it would be.

Anticipate and Dodge Tentacle Slams

Tentacle Slams are easily avoided by paying attention to which direction Patient 13 raises two of its tentacles. It will always slam both tentacles vertically downwards and then briefly sweep them to the side.

If you're under its tentacle, you'll have a short time to move away before taking a lethal hit that can instantly down you. Moving as far away as possible from the tentacle slam is the safest way to avoid any sort of deadly damage.

Destroy Toxic-Shooting Tentacles

Shoot the Tentactles Yellow Spore

Several tentacles will emerge out of the water from a distance and will fire toxic projectiles at you. You can take a tentacle out by shooting the yellow spores on its body.

Use Cymbal Monkeys or Decoys to Distract Zombies

Patient 13 Zombies

Zombies will continuously spawn during the fight. If you're a solo, you'll want to bring Cymbal Monkeys and Decoy Grenades to lure zombies away so that you can focus on the boss.

Second Phase

Shoot the Mouth

Shoot the Mouth 2nd Phase

The second phase begins once Patient 13 reemerges out of the water, seemingly covered in molten lava. At this point, the mouth will now be the main weakness. Shooting it may be difficult as the boss will frequently dive up and down the water.

Third Phase

Aim for the Eyes

Shoot the Glowing Eyes

Patient 13's glowing blue eyes are its weakness in the third and final phase of the fight. You may have to keep strafing left and right while shooting it to avoid getting hit by tentacle slams.

At this stage, the arena will also be cut into two sections as two tentacles will block half of the arena. You can go from one half of the arena to the other by jumping down the water and climbing up the ladders on both sides.

Avoid the Tongue

Patient 13 Tongue Drag

In the third phase, it will stick out its tongue and drag anyone that touches it into its mouth. This results in an instant down, so avoid going near it once it sticks it out!

Dive into the Water During Shockwave Attack

Patient 13 Shockwave Attack

When you see Patient 13 charging up electricity and once blue particles start appearing on your screen, quickly dive down the water to avoid an impending shockwave attack. One shockwave attack can potentially down you in one hit.

Use the Chopper Gunner to Deal Damage Safely

Chopper Gunner Patient 13

The Chopper Gunner is extremely effective during the chaotic stage of the final phase since you'll be invincible the whole time and you'll be able to deal damage safely.
