To fight Huang Gai in Battle of Chibi, wait for Guo Jia to mention that the "winds have begun to shift" before heading to the yellow flag. Upon arriving, a quick dialogue will occur and a fight with Huang Gai will intiate afterwards.
The requirement to achieve the true ending is to defeat Huang Gai before the altar is lit, thereby altering the fate of Wei.
Wei True Ending Guide (Cao Cao's Forces)

Utilize the War God Rage to deal higher damage and gain access to the True Musou Attack. While occupying the southern bank and waiting for the winds to change, it is highly recommended to preserve your musou gauge for Huang Gai.
Additionally, the Spirit Pill item can be used to restore the Musou gauge. This may be slotted in the portable items to enter War God Rage sooner, or land a few musou attacks before or after the War God Rage state.

Likewise, switching to a companion and utilizing their War God Rage can also be used to expedite the fight. Switch to a companion, enter War God Rage, spam battle arts, and execute the True Musou Attack for a quick burst of damage.
How to Use the True Musou Attack

Remember to make use of tactics during the fight. The Mounted Charge tactic can deplete a high amount of fortitude, allowing more assaults without relying on continuous attacks and timely parries.

Select battle arts, weapon combos, and performing assault can launch Huang Gai and throw him overboard the ship. It is imperative to avoid this instance as Huang Gai's reset and return to the ship will take a lot of time, delaying his defeat.
Keep in mind to keep him grounded, and position your assaults and musou attacks where his knockback will land within the ship.

When Huang Gai emits a red glow, he will charge towards the active character for a grab attack.
Pause your attacks shortly when he attempts this move, and avoid accordingly. Getting hit will subject you to a long animation and massive damage, wasting time and putting Wanderer/Companion near death.
Once Huang Gai's health is critical, he will take a stance and unleash his Musou attack. The move starts after a red silhouette flashes above him.
The move cannot be interrupted, hence, it is best to focus on timing your evade or take advantage of War God Rage's invincibility to avoid this move.
Equip Luan Weapons or at least Grade 7 weapons to defeat Huang Gai quickly. His battle is a race against time, hence, equipping the strongest weapons in your arsenal is a must to defeat him before the altar is lit.
Luan Weapons are the strongest weapons in the game, boasting the highest attack stat and equipped with powerful traits. If Luan weapons have not been obtained before challenging Huang Gai, Grade 7 Weapons purchased from shops post-game can suffice is an alternative.
If players are still struggling to defeat Huang Gai—especially when Luan Weapons are not available—, it is recommended to lower the game difficulty down to Historian to experience an easier encounter.
Historian enables all enemy attacks to be blockable and allows the Musou Gauge to increase at a faster rate, making the fight a breeze to clear.

Huang Gai is a loyal member of Sun Jian's forces. He is available as a companion, and can be encountered as a key enemy in unlocking Wei's true ending during the Battle of Chibi when aiding Cao Cao's Forces (Wu).
During the Battle of Chibi, he starts as an ally of Cao Cao's forces but will later defect and set fire to Wei's ships. In Wei's true ending, his defection is impeded by Wanderer, allowing Cao Cao to occupy the southern bank.
