How to Use Supremos
Unlocked During Du or Die
You'll get your first Supremo near the end of Du or Die Operation Mission. After going back to Clara's Camp with the Depleted Uranium, Juan will give you the Exterminador.
From here, you can purchase different Supremos from Juan, though most of them have unlock requirements like ranking up or completing a Yaran Story.

Using Supremos in Battle
After equipping a Supremo through the Arsenal menu, you can trigger its unique ability by pressing L1 + R1 (for PS4/PS5). Each Supremo has a different ability so make sure to equip the right one for your current situation!
Takes Time to Recharge
Supremos, as Juan said, are still in experimental phase. It will need time to cooldown before it can be used again in battle.
Taking out enemies reduce its cooldown time, effectively allowing you to spam its ability when you're on a killing spree.
