The secret ending can be obtained by collecting three Mysterious Circles scattered in Thedas:
These Mysterious Circles unlocks a codex entry when acquired. Having all three codex entries before the point of no return unlocks the secret ending.
Note that it is not required to attain the true ending to unlock the post-credits scene.
Solving the Waterfront Glades puzzle in Arlathan Forest grants access to the first Mysterious Circle. Rook will hear voices of unknown origin upon acquisition, foreshadowing the Executors revealed during the secret ending.

After defeating The Formless One in Pinnacle of Its Kind, head to the northwest room to find the second Mysterious Circle. The voices will appear again, expressing astonishment on Rook's ability to find clues of their existence.

After defeating the Revenant Dragon, the final Mysterious Circle can be found in a corner near the chest containing The Last Resort unique ring.
After collecting all codex entries, the three images connect to form the Executors' symbol that appears in the secret post-credits scene.

The Storm Quelled achievement can be unlocked upon watching the post-credits scene after collecting all three Mysterious Circles.
Trophy Guide and List of Achievements
The secret post-credit scene showcases a big revelation regarding the past events in the Dragon Age series. It reveals that a mysterious group of hooded figures— theorized to be the Executors —had a hand in instigating the major events from previous games. These include: the theft of the Lyrium Idol in Dragon Age II, Loghain's betrayal in Dragon Age: Origins, and Corypheus' "Breach" creation in Dragon Age: Inquisition.
The secret ending lays out the plot and possibly the next major villain for the series' following installment, implying that the mysterious faction have finished their preparations and are done hiding in the shadows.
The codex entries talk about the discoveries of two elves who stumbled upon a Mysterious Circle. Each encounter they have with a circle is evidently welcomed by greasy air and sheer cold, in addition to inexplicable unease. Each circle is said to contain white markings and black triangles, identical to the symbol revealed during the secret ending.
Upon stumbling on the third circle, they described an encounter with a figure donning a grey robe, a grey helmet, and a mask. These figures appeared in the post-credits scene as well, whispering and guiding the main antagonists of the previous installments. These unknown figures are implied to have orchestrated the major conflicts from the entire Dragon Age franchise thus far, though their goal remains unknown.
In the codex entries, the Keepers spoke of the circles as relics of the olden days of Elvhenan. They mentioned the unease felt by the author is due to the artifact being of "evil" origin, as the followers of the Forgotten Ones were often categorized as such. The author disagreed, however, and described that the circles feel "wrong" rather than "evil". They sought to investigate its existence and uncover more of its secrets afterwards.
In Dragon Age: Inquisition, the same white markings over a black inverted triangle appeared in the Inquisition outposts, prompting the Inquisitor to either request for the cooperation of the Comtesse, or capture the vandal via a decoy. Regardless of the action taken, the vandal refers to their faction as the "powers across the sea", hinting that the mysterious faction is of foreign origin.
