Players should spare Lady Oichi if they wish to romance her in Assassin's Creed Shadows (AC Shadows). Read on to see the different outcomes and how it affects your relationship with Naoe and Lady Oichi.

In The Wheel Unmasked, it is revealed that the Mourned One is Lady Oichi. Choosing to spare her will cause Naoe to storm out in anger, ending the quest after some dialogue. This will also allow Yasuke the option to romance her in the upcoming quest, The Wheel Turns.
Attacking Lady Oichi as Naoe will cause Kuma to jump in the way of the blade, saving Lady Oichi. Naoe, quickly regretting her actions, will storm out of the temple. This spares Lady Oichi regardless of your choice.
Whether or not you choose to spare Lady Oichi, she will give Yasuke the sword that belonged to her brother.

Alongside the Myth Slayer Katana, these rewards are granted to the player regardless of whether they spare or attack Lady Oichi.
Players should spare Lady Oichi as attacking her will still result in Kuma taking the hit for her. Attacking her will also lock Yasuke out of the quest to romance her in The Wheel Turns.
