Sam's skin will visibly redden and appear patchy when he gets sunburned due to regular, especially prolonged, exposure to sunlight. This is most noticeable during extended periods without Sam resting in a private room.

Getting sunburned has no effect on Sam's stamina or health, nor does it have any lasting repercussions. It is purely an aesthetic game detail that adds to the immersive experience of players throughout Sam's journey, much like the frostbite in Death Stranding 1.
