Do Order No. 026
Before you can attempt to unlock the trophy, Written in the Stars, you'll first need to progress the main story until you get to Order No. 026. Once you find The Motherhood during this mission, you should see the Miracle Spring outside the building. This is where you can attempt to get the Written in the Stars trophy.
Bathe in the Miracle Spring
Stand on the Miracle Spring near The Motherhood in Australia and hold Square to Bathe. Note that it must be night time when you are trying to get the achievement so Rest and pass the time until it's night time.
Hold L1 to Look at the Stars
Hold L1 to look at the stars. You will notice that there are multiple constellations at the sky. Look at any of them and they will form an image (like Lou, Cryptobiote, Kojima Productions logo, etc.). This will unlock the Written in the Stars trophy.
Written in the Stars Rewards
No Rewards Aside From Temporary Boosts
Aside from unlocking the Written in the Stars trophy, there are no in-game rewards given to Sam. Although, you will get temporary boost on some of your stats depending on the hot spring that you've used.
