Setting your birthday in Death Stranding 2: On the Beach will allow you to view a birthday greeting from the cast while playing during the chosen date. Read on to learn the best birthday to choose, what your birthday does, and what the birthday effects are during gameplay.

What Does Your Birthday Do?
Code for Motherhood Door in Order No. 026
One birthday effect tied to gameplay appears in Main Order No. 26. At the final delivery point for this order, you’ll need to enter a specific door code to access the Motherhood terminal and complete the order. The code is the date of birth you entered at the start of the game.
This is currently the only birthday effect that directly impacts story progression since the birthday code is required to complete the order.

Sets the Date for a Birthday Greeting Easter Egg
Your chosen birthday in Death Stranding 2 only serves to determine when you'll receive the Birthday Greeting cutscene from the game's cast of characters.
While playing during your in-game birthday, you'll get a birthday greeting from the crew of the DHV Magellan as well as a pizza delivery from Higgs (a nod to his delivery missions from the first game).
No Other Birthday Effects During Gameplay Despite the Lore Drop
Despite the game's lore drop about how your birthday affects your susceptibility to developing DOOMS, your chosen birthday has no effect on Death Stranding 2's gameplay.
Since Sam's DOOMS level is set based on the story, choosing a birthday that falls under the constellations of Cancer, Pisces, Cetus, or Delphinus will not make any difference to his abilities in-game.

Death Stranding 2 (similar to its predecessor), seeks to make connections with its players. To add a personal touch to your playthrough, you can opt to set your in-game birthday to your actual birthday so that you get the birthday greeting at a meaningful date.
Choose a Birthday Near Your Play Date
If you're concerned about data privacy (especially since the game technically has multiplayer features), you can simply set a random date closer to your starting play date so that you can experience the easter egg without sharing any of your personal data.
