Centipede Guai can be found in Webbed Hollow, Middle Hollow. Starting from the Shrine there, take the path on the right side of the Shrine where you'll see eggs. Just head straight and drop down to the large open area. Once you're down the area, the Centipede Guai will drop down, and the fight will start.

The Centipede Guai is fairly easy to beat due to its low HP and defenses. Aggressively chaining together spells, vessels, and attacks will quickly melt the boss' HP, especially at the start of the fight.
The Centipede Guai has an attack where it summons small creatures that roll towards you and explode upon contact. The best strategy to deal with them is to go near a wall and dodge them as they roll towards you so they collide with the wall instead.
The Centipede Gaui has an attack where it rolls up into a ball and charges at you multiple times. The best strategy to deal with the attack is to simply dodge it as it charges towards you.
Moreover, it's best to avoid using spells or even attacking it while it's rolling, since you'll just get hit.
