Items such as Tonifying Decoction can be obtained by opening chests and containers that are scattered around the world. Simply move close to the chest or container, then open them.

Defeat Enemies and Bosses

Items such as Tiny Piece of Gold can be obtained by defeating enemies. On the other hand, items like Blood of the Iron Bull, Mind Core, and Tadpole can only be obtained by defeating certain bosses, making them quite difficult to obtain. However, such items are more powerful compared those that can be obtained simply by defeating regular enemies.

To upgrade or craft your equipment, interact with a Shrine, select Craft Weapon or Craft Armor, then choose the weapon or armor you want to craft.
