Tough Mangoes are most commonly obtained by completing the Minigame Mango Rush. The amounts you obtain are dependent on the level you played, and amount of mangoes you were able to harvest.

Sell to a Shop
Tough Mango can be sold to a merchant in Shops for 8 Rupees. If you have an excess and don't need them for any smoothies, selling them off can be a decent way to farm some extra rupees.
However, since the Tough Mango ingredient cannot be bought from a merchant, be sure to keep a few around, at least until you have created all of the smoothies it is used as an ingredient for.
Can Also Be Consumed As Is
The Tough Mango ingredient can also be consumed as is to restore 1 Heart in a pinch. However, you will get much better use out of it by blending it into a smoothie, so it's better to rely on other methods of gaining HP, such as creating a Bed Echo and resting in it.
