Once you have the required materials and Infamy, speak to a Blacksmith at a pirate den or outpost and you can start crafting your weapon.

Skull and Bones - Install on Your Ship

Install your newly crafted weapon to your ship on the correct slot by going to the Manage Ship menu while docked at a pirate den or outpost, or from anywhere if you're on foot while in these places.
