The Dead Space Remake (2023) will have a secret ending included in its New Game Plus feature. This can only be unlocked by collecting all 12 Marker Fragments and performing a ritual. Read on to find out what happens in the secret ending.

Complete Base Game Start New Game Plus Collect 12 Marker Fragments Put Marker Fragments Back in Crew Deck Complete the Game as Usual

Unlocking the Secret Ending is only doable once you have completed the base game and unlocked New Game Plus. Start the story as you normally would, but play close attention to the existence of Marker Fragments as you complete the objectives of each chapter.
Once all 12 Fragments are found, return them to the Crew Deck. Find Captain Mathius' Room in the Executive Quarters and place the marker fragments back on their pedestals.
Complete the game as you normally would, and wait for the alternate ending to occur. You will still be awarded 50000 Credits and 10 Power Nodes but we won't spoil what's got Isaac smiling there.

When getting the secret ending, Isaac's voice line changes during the Hive Mind boss fight saying that something showed him what he has to do.
This voice line shows a completely different Isaac who's original goal is to simply return the Marker in Aegis VII which has already been done.
Unlike the original ending Isaac is now directly influenced by the Marker and is experiencing a full Schizophrenic disorder seeing as how he seemingly believes that Nicole is real.
Although the Secret Ending sets a nice tone for Dead Space 2 because of how it teased that Isaac will build a new Marker, there are a few disconnections compared to the original plot of the series.
One of these would be Isaac's unwillingness to have his mind extracted Dead Space 2 for the Marker Codes, which is required to build a new marker.
Another would be Isaac's interactions with Nicole in Dead Space 2, as Isaac is aware that she is not real and is constantly trying to get rid of her apparition.
If we are to be blessed with a Dead Space 2 Remake, following this Secret Ending will change the main plot of the series.
The first change would be that there's a chance that Isaac can either go straight to Earth where he builds a new marker for convergence, or go to Titan Station where the original Dead Space 2 took place and ally himself with the Unitologists there.
Another change would be Isaac's relationship with key characters such as Ellie Langford and Nolan Stross, as Isaac befriended these characters when he planned to stop the infection in Titan Station.
