There are 5 difficulty tiers available in the game, with the hardest difficulty being the Impossible level, which was previously only available after beating the game, but is now available from the onset in the remake. The Story difficulty is new to the remake.

Necromorphs Deal More Damage in Higher Difficulties

Going up in difficulty levels increases the health values and damage output of each Necromorph you fight. Players will have to be increasingly cautious during Necromorph encounters in higher difficulties, as a few hits from even the weakest Necromorphs can quickly drain their health bar.

Permadeath and One Save Slot in Impossible Difficulty

When playing on Impossible difficulty, players are restricted to a single life and save slot, meaning they cannot die at any point in their playthrough or create multiple saves. Due to these constraints, players will have to consistently stay alert and play optimally to prevent accidental or surprise deaths throughout their playthrough.

Is There a Difficulty Trophy?

Difficulty Trophy Is Available

Gold G:150	Untouchable
	Complete the game on the hardest difficulty setting. ►Impossible Mode Rewards and Changes

Completing the game on the hardest difficulty level, Impossible, will reward the player with the Untouchable trophy. Obtaining this trophy is only possible from a fresh save, as you cannot change to Impossible difficulty during an on-going playthrough.

How to Change Difficulty

Change Difficulty in Gameplay Settings

You can change your difficulty at any time during your playthrough by opening up the Gameplay tab of the Settings menu and choosing the Select Difficulty option. In the difficulty selection screen, you can freely swap between Story, Easy, Medium, and Hard difficulties.

Impossible Difficulty Only Available from New Game

However, you cannot swap to Impossible difficulty in the middle of an on-going playthrough. This difficulty mode is only available when starting a New Game due to its permadeath and single save slot restrictions.
