Update: After finishing the game (Dream), I started ng+1 and checked the offering box: the axe is there, so much so that it gave me the objective of collecting all the weapons.
Can confirm, if you open chest and die the item will be permanently missing.
same I died after opening the chest and boom i dont see the weapon anymore.
Fun fact. I died while I was opening the chest, thanks to the Goblin (I was blind, in every meanings). The result? The chest is open, but no axe in my inventory. And I check it many times.
