Speak with Jadwiga in her workshop and choose the option to transform your gear. Her workshop is located to the right of the elevator when facing it in the MagMell Institute.
Upgrading your gear to higher levels requires higher-tier materials. You can buy Customer Tokens from Jadwiga's Minions scattered across the map to unlock and purhase higher-tier upgrade materials from Jadwiga's general store.

Transforming your equipment changes which attributes it scales from. This is especially useful if the Blood Code you have equipped has high values in certain attributes and you want your favorite weapon to scale off them.
You can also reduce your gear’s burden, at the cost of lower stats and attribute modifiers.
Attributes Guide: Status Explained
Your weapons and defensive formae can be imbued with elements and status ailments. Transforming your weapons with an element adds that element’s damage to your weapon, while imbuing them with a status ailment causes your weapon attacks to inflict that ailment.
Transforming your defensive formae will increase their base defense against a certain element or status ailment, while their guarding defense offers full negation of that element (but not status ailments).
