Collect All 20 Mask Shards
There are 20 Mask Shards to collect in Hollow Knight: Silksong. Collecting 4 of these completes a mask and increases Hornet's maximum health by 1. Collecting all of them gives you a total of 10 Life Points.
Collect All 18 Spool Fragments
There are a total of 18 Spool Fragments to collect.
Picking up two Spool Fragments increases the Spool below Hornet's health by one segment. This allows you to use more Silk Skills or Heal more frequently.

Collect All 3 Silk Hearts
A total of 3 Silk Hearts to collect in Pharloom and are obtainable after defeating specific bosses. Silk Hearts automatically restore Silk once it reaches zero.

Defeat All 43 Bosses
Achieving 100% completion includes defeating all story and optional bosses in the game. This also includes bosses that can be skipped such as Moorwing.
Obtain All 6 Silk Skills
There are 6 Silk Skills in total to find and collect. These give you special attacks that help with defeating enemies and bosses.

There are a total of 8 Abilities to collect and improve Hornet's platforming abilities. These are obtainable as you progress through the main story and is required to get 100% completion.
All Ability Locations and Upgrades
Obtain All 7 Crests
A total of 7 Crests are available to collect and use. These let you change Hornet's movesets and allow her to equip tools.
Unlock Vesticrests to Equip More Tools
Aside from the slots allowed by your equipped Crest, the Vesticrest will provide two extra slots to equip tools which remain equipped regardless of your currently equipped crest. To find the Vesticrest, players will need to speak to Eva at Weavenest Atla.
Obtain All 59 Tools
Tools are equippable trinkets that offer Hornet activated abilities and modifiers. These can be obtained after defeating specific bosses, from well-hidden locations, and purchased from NPCs such as Shakra.
Collecting all Tools is required to get 100% completion.
Rescue All 30 Lost Fleas
Although you only need to rescue 5 Fleas to complete the The Lost Fleas wish, rescuing all 30 Fleas gives you both the Flea Finder and Fleafriend trophies and is required to get 100% completion.
Fulfill All 54 Wishes
Wishes serve as the game's side quest system and you can earn rewards such as Crests, Rosaries, items, and achievements. Fulfilling all of these Wishes is required to get 100% completion.

Obtain All 27 Maps
There are 27 map pieces that can be obtained as purchasable items from Shakra. Obtaining maps slowly fills up the map screen, allowing you to navigate Pharloom easier.
Unlock All Bellways and Ventrica Stations
Bellways and Ventrica Stations are areas in the game that serve as fast travel points. You need to defeat the Bell Beast first in The Marrow to unlock Bellways, and Ventrica Stations can be activated in the Citadel and Underworks.
Note that most stations require you to pay Rosaries before unlocking them for fast travel.

A bit of this is incorrect. Bosses, maps, wishes, any travel do not count toward the percent. You have to complete/defeat most to get things that are required but you don't have to do them all. You don't need to fight Sister Splinter, Lace 1, Last Judge, Fourth Chorus, Tormented Trobbio, Pale Stag, Clover Dancers, Lost Garmond, Watcher at the Edge, Crawfather or Moorwing. You only need to buy all the maps Shakra has. You don't need to do any wish that doesn't give you a mask, spool, or tool.
