Would also be nice if there was mention of if the enemy is missable at all, i.e. Gormand and Zaza, and Shakra. Also, if they contribute to the Hunter's Memento/trophy or not, which I've read that the aforementioned two missable entries don't contribute to it. I think there's a third one, but not sure.
Really need to add a column to mention the (expected) minimum Act to encounter the enemy. I say expected to rule out the argument of "if you do this glitch/secret, you can encounter this enemy early" etc.

during act 3, stay on the bench in bellhart and quit the game, then come back. youll get the summons
How are you supposed to get the Craw Summons, needed for Crawfather, when one of the journal entries needed for the Summons is exclusive to the Crawfather fight??

The hardbone hopper is found in Deep Docks
