Attaining a 100% exploration status and getting the Explorer achievement mainly involves getting all the collectibles in the game! Below is a list of all the types of collectibles, landmarks, and quests that you have to do to obtain a 100% exploration rate!
Viewpoints

A staple of every Assassin's Creed game, viewpoints are high places in the district that you can synchronize with to reveal a bit of the map and collectible markers within your vicinity in the district. It's recommended to synchronize with every viewpoint first before looking for collectibles within that district, so they're all marked on your map. Happy hunting!
Gear Chests

Gear chests contain outfits, weapons, and upgrade schematics. They're the golden chests found throughout the city. Simply walk up to one and toggle the prompt to Loot Chest, in order to claim what's inside.
Lost Books

Lost Books are collectibles that you don't get rewards immediately upon collecting them from the world, but you can give them to Al-Jahiz for some potential rewards. Lost books just look like ordinary books, but they will glow yellow with your Eagle Vision and they are also marked on the map.
Enigmas

Enigmas are puzzles, riddles, or maps that you have to solve to find a treasure location. These treasures are either outfit dyes or talismans. To view the enigma, go to your Bag in the Inventory.

Dervis' Artifacts are collectibles that you'll have to pickpocket from NPCs. The NPCs themselves will be marked on the map or through Enkidu, then you'll just have to walk up to them and successfully pickpocket them to claim the artifact.
Collecting all Dervis' Artifacts will reward you with Treasure Hunter outfit and the Curio Collector achievement.

Historical Sites are collectibles that appear as glowing orbs in various locations around Baghdad. Interacting with these glowing orbs will update your Codex with information about the site's historical significance or any related facts and trivia about that area of Baghdad.
Mysterious Shards

Collecting all Mysterious Shards will allow you to obtain the Legendary Outfit Milad's Outfit and Legendary Weapons The Samsaama and Shamsir-e Zomorrodnegar. You have to either pickpocket Mysterious Shards or assassinate NPCs marked on the map.
After collecting all Mysterious Shards, you will be able to place them in the underwater chamber in the Northern Oasis as part of Nehal's Calling Case.

Tales of Baghdad are short side quests that count towards 100% completion of the district. Simply talk to the NPC to start the side quest, then complete it to check it off the list. 3 Tales of Baghdad quests reward Skill Points, while the other 3 reward money.
