Which MGS Game to Pick

Picking the MGS 3 option for a first normal playthrough is a great choice since the bonus Flecktarn camo you will receive prevents battery drain, allowing you to use items that need battery power, like the different sensors, Thermal Goggles (useful in collecting plants and animals to unlock EZ Gun for NG+), and NVGs, infinitely. Flecktarn also has a high Camo Index in jungles, forests, and foliage, making it useful to a lot of locations during early game.

If you're planning to get a high title from the get-go, choose MGS 1 since Auscam helps mitigate damage taken, which is a pre-requisite for high ranks like FOX and FOXHOUND.

Pick Based on Your Preferred Bonus Equipment

Alternatively, you can simply pick based on the initial equipment you'd prefer to start with. Some camos, like Grenade, grant you unlimited grenades when equipped, while Desert Tiger allows for unlimited use of an equipped suppressor, making for an interesting run.

Each initial equipment has a unique effect that is beneficial to gameplay, so choose based on which you think is most useful for you on your first playthrough.

All Question Unlocks are Available after Finishing the Game

After clearing the game at least once, all other MGS question unlocks will be permanently available for any subsequent New Game+ playthrough. Therefore, whatever your initial choice is, it will not lock you out of these rewards.
