The Pain is the second boss you fight in Metal Gear Solid Delta Snake Eater (MGS3 Remake). Read on to learn how to beat The Pain in Chyornaya Peschera Cave, including tips for getting a non-lethal boss kill, missable collectibles you can get in his arena, and rewards for beating him.

Headshots are very important for this fight if you plan to go for a stamina takedown. Take your time and go for headshots when using the Mk22 since 2 consecutive headshots take away 1 whole segment of his stamina bar.
If you're going for a kill instead of a non-lethal takedown, switch to the AK-47 instead since it chunks away at his HP much faster. This method has the added bonus of not needing headshots.
Throughout the fight, the Pain will encase himself in bees, making him immune to any type of damage. To remove the bee shield throw 2 grenades (normal or WP work great) or hit him with 2 M37 shotgun blasts.
Of the two methods, grenades work best since he gets momentarily stunned when the second grenade hits. During this short stun window, you can comfortably drill him with headshots.
If you're playing on Extreme, prepare to make your shots count since you'll need 4 shotgun blasts to remove his bee shield instead of the normal 2. There's a respawing M37 ammo drop nearby, but you'll have to swim to the other platform in the arena to get it.
If you see bees swarming around the Pain and that he has no bee shield, don't throw grenades at him. If you do, his bees will catch your grenades and ferry them back to you, return-to-sender style. If they do catch your grenades though, just shoot the small bee swarms to get them to drop the grenades.
Note: If you're using the Mk22 to get a stamina kill, be aware that shooting bee swarms carrying grenades with it will not work. You'll have to switch out to a weapon that shoots bullets to stop the bee swarms.
If you get hit by the Pain's hornet serum throwable, hornets will start swarming and attacking you. To avoid taking damage, you can jump into the water and go for a short swim.
Note: Don't listen to the game's tip of using Bug Juice. It's a hit or miss with that consumable. Instead, if you have Smoke Grenades, chuck one at The Pain to confuse his green hornets whenever they're out and harassing you.
Throughout the boss fight, the Pain will comically announce that he's about to shoot you by shouting Tommy Gun! When he does this move, dive into the water and hide behind any of the platforms to dodge his shots.

When the Pain drops to half HP or half stamina, he'll start firing off Bullet Bees that leave red trails. If you get hit by a Bullet Bee, Snake's HP will continuously go down if left untreated.
To cure a Bullet Bee wound, open your Survival Viewer and go to Cure. Then, select the Bullet Bee injury and use the Knife, Disinfectant, and Styptic to treat it.

For non-lethal takedowns, you're best option will be the Mk22 tranq pistol. Go for headshots to make the most out of your ammo. If going lethal, the AK-47 is the best option (it's why the entire cave system is littered with AK-47 ammo).
To remove his bee shields, you can use Grenades or shots from the M63. The effectiveness of either weapon depends on your chosen difficulty.

Defeating the Pain non-lethally will reward you with the Hornet Stripes Camouflage. When equipped, it prevents bees, spiders, and leeches from attacking you.
