Sleep in the Bed Again to Collect the Ghost Medal

Super Mario RPG Remake - Sleep in the Bed
After you collect the flags, go back to Monstro Town and sleep again in the same bed where you started the quest. The Three Musty Fears will then reward you with the Ghost Medal; it is an accessory that will be automatically equipped to Mario after the quest, replacing his current accessory.

The Ghost Medal is an accessory that can be equipped by everyone, and its effect is halving damage taken from all attacks that deal numeric amounts of damage.

Sleep in a Certain Bed in Monstro Town

Super Mario RPG Remake - Three Musty Fears House
In Monstro Town, the house to the right of Goomhilde's shop has a bed inside it. Beside the bed, there's a sign beside it written by the Three Musty Fears, telling you to feel free to sleep on the bed. Turn off the light then interact with the bed to sleep.

As you are sleeping, the Three Musty Fears will visit Mario and invite him to play their 'Find the Flag' game, starting the quest.

Flags Won't be There if You Haven't Started the Quest

If you haven't started the quest yet, the flags won't be in their respective positions. The Three Musty Fears will only hide the flags after they have interacted with you.
