Complete the Underworld Main Quest Cannot Save in Restricted Districts or in Combat

Complete the Underworld Main Quest

You can't manually save the game until after the 4th Main Quest Underworld and at the start of New Tricks. Until that point, you'll just be relying on your autosaves.

Starting from the next quest, the New Tricks main quest, you can start saving via Save Game in the Pause Menu.

Cannot Save When Sneaking in Restricted Districts or in Combat

You can't save during combat or when inside certain districts—specifically, when sneaking or when the “Do not get caught” instruction appears in the top right of the screen.

You Can Save Scum to a Certain Extent

For the most part, you won't be able to save directly before a choice scenario occurs. A lot of the choices that are prompted to Kay take place within faction districts after a fair bit of sneaking around and/or combat. To save scum those choices, you'll have to load back into a save that likely takes place outside the restricted area/faction district (in short, you may need to do all the sneaking and fighting again before getting back to the choice scenario).

Still, overall, it's possible to reload into an earlier save and make a different choice for a better outcome. Even while sneaking; if you get caught, you can reload back into the last autosave (which should reload you back to the start of whatever room/area you're in).
