The Blood Specimen (Denatured) can be found in the Blood Lab alongside the Blood Collector. To solve this puzzle, just click the middle square, and this will give you the ability to craft the Handgun Ammo and Hemolytic Injectors.

The Blood Specimen (Converged) can be found inside the Closet in the Waiting Room on the East Wing.
You will need to get the ID Wristband (Level 1) to unlock the office, which can be found on the dead researcher's body on the 2nd floor of the East Wing, just past the Lead Researcher's Office. Note that this is where you will encounter the Chunk boss fight, so either lure it away or kill it before getting the wristband.
To solve the puzzle, click the top middle square first and then the second square from the bottom. This will let you unlock the ability to craft Med Injectors and 12.7x55mm Ammo.

The Blood Specimen (Reversible) is also found in the Blood Lab but is locked behind a security door that also requires the ID Wristband (Level 1), which can be found on the dead researcher's body on the 2nd floor of the East Wing, just past the Lead Researcher's Office.
To solve the puzzle, click the bottom-left corner square first, and then the middle square, then click the top-right square, and then the second square from the top one. This will let you unlock the ability to craft Steroids and Stabilizers.
Blood Specimen (Polymerized) - Insanity Only

The Blood Specimen (Polymerized) can be found next to the Pantry Key inside the Closet in the Waiting Room. However, this blood specimen is only available in Insanity difficulty.
To solve the puzzle, click the second square on the left first, then the second square on the right, and lastly the second square on the middle. This will let you unlock the ability to craft the R.I.P. Knife and Handgun Ammo x30.
How to Solve Analyzer Blood Specimen Puzzle
Transform Every Square Nodes into a Circle
You will need to transform every square node into a circle node to complete the puzzle. However, you can only click square nodes, which will transform the connected nodes into either a square (if they are a circle) or circle (if they are a square).
