Unfortunately, despite Grace's pleas to spare the ballooned baby zombie, you're required to shoot Emily in order to progress the main story. There are no consequences for shooting and taking down Emily in her zombie form.

What Happens After Killing Emily?

Spoiler Warning The next section details events that occur after killing Emily in Resident Evil Requiem. It will contain major story spoilers beyond this point!

Grace Goes Along with Zeno Leaving Leon Behind

Grace, incensed at Leon for putting down Emily, will leave him behind and instead join Victor and Zeno (an agent of The Connections) when they tell her they're taking her to Elpis. This marks the big story shift from Grace to Leon's PoV in Raccoon City.

Emily Survives if You Choose the Good Ending of RE9

The Good Ending

If you chose to Release Elpis at the ARK, you'll trigger the good ending of RE9. When all is said and done, Leon will remark that he did not aim for vitals when he shot Emily, stating that Grace can still save her with the newly discovered Elpis antiviral.

Later on in the same ending cutscene, you'll see a news broadcast showing police vehicles and an ambulance arriving at the Care Center in Rhodes Hill. Plastered on the screen will be a headline saying that a "young girl [was] rescued from...Rhodes Hill".

In a post-credits cutscene, Grace's desk will feature a new picture of her and Emily, signifying that Emily lived after receiving a dose of Elpis and that Grace was raising her similar to what Alyssa did for her.
