Choosing whether to cure Chilliam first or pursue the Extension Corps does not affect the outcome of the main story.
The only difference is that you get the UFO Spin Bros. Move upgrade earlier if you choose to cure Chilliam first. On the other hand, choosing to pursue the Extension Corps allows you to obtain the Ball Blitz Bros. Move upgrade earlier.

Yeah most split paths just determine what upgrade you get sooner or some aesthetic like which sibling runs the shop. The one that makes arguably the biggest gameplay difference is choosing to let Peach or Luigi get kidnapped and only because of EXP management.
