You can obtain the Pencil by grabbing it from the Office Table in the Chairman's Office, West Wing of the Care Center. Be careful when entering the room, there will be zombies inside that will attack you if you are noticed.

How to Use the Pencil

Pencil Overworld Location

After getting the Pencil, go to the coffee table in the Chairman's Office receiving area and interact with the notebook. Use the Pencil and swipe around the page until the page is darkened and the code for the Moon Puzzle Box is revealed.
