Play Moleville Blues to Toadofsky

Super Mario RPG - Moleville Blues Notes

You get the Tenor Card by playing the Moleville Blues song to Toadofsky, unlocked only after saving Dyna and Mite. You'll need to have acquired the Alto Card as well.

The notes for Molevile Blues are Mi Do So Do Re La Ti Do.
