Otoroshi is a large yokai that mainly attacks straight ahead with its arms. Ninja style's mobility makes it easy to dodge its wide attacks and slip behind to counterattack.
Equip the Ninja skills Yokai Banisher and Evil-Dispelling Evade to purify Yokai Realms that hinder your Ki regeneration.
Position yourself at the rear when attacking for the extra backstab damage while in Ninja style.
Beware of two attacks that can still hit you with its back turned: the ice shards and when it swings two icicles in a circle while in the Dark Realm.
In the Dark Realm, Otoroshi will use birds as projectiles, leaving its chest exposed. Use Ninjutsu such as kunai and shuriken to break the eggs and inflict massive damage to its Ki.
Otoroshi's red attacks are fairly predictable because of the lengthy wind-up, so always attempt to counter with a Burst Break.
If you're still struggling with Otoroshi, don't hesitate to farm some levels and equip better gear before trying again.

Summon an acolyte from nearby benevolent graves to serve as a distraction and help deal chip damage. You'll need Ochoko Cups to summon acolytes, which you can obtain by defeating revenants from bloody graves.
Use talismans and items such as Power Talismans and Power Pills to boost your attack. Note that they do not stack and only refresh the duration of the attack buff.
