The Combination Lock Dial is the last item you need to open the Shrine Vault in the Dark Shrine Path during Chapter 2.

Note that placing the Combination Lock Dial will solve the Shrine Vault puzzle, so be sure you have all the items you need in the area before leaving.

Head west of the Dark Shrine Path during Chapter 2, and look for a Crane Ema. Flip this Ema, and the Combination Lock Dial should be attached to its back.
