The doll in Silent Hill f is likely the Tsukumogami, the God seen in the bad and true ending. It often appears advising Hinako of the potential dangers in the story, and appears as an ally to save her owner from peril.
It is first encountered in the first cutscene, where a young Hinako is seen playing with the doll. It reappears later, warning Hinako of Rinko, then warns her of Fox Mask. It is seen again during Hinako's vows in the Dark Shrine, and in asking for assistance in helping Shimizu Hinako, hinted to be her original self.
Endings Explained and Conditions

The doll's advice against Fox Mask and in reminding Hinako of her name suggests that it does not want the wedding to occur. This is further supported by its final message to help Shimizu Hinako, known to be her original self, as seen in her Journal entry.
Note that Shimizu Hinako only survives in the good ending, where Hinako triumphs over Shichibi. However, the post-credits scene reveals the geysers gaining activity in Ebisugaoka once more, likely a hint that the Tsukumogami only desires the downfall of the Fox Clan regardless of Hinako's well-being.
The Sennensugi Shrine served by Sakuko's family is known to practice the Tsukumogami Faith as seen in the shrine's Journal entry. The faith revolves around worshipping the divinity descending on the oldest objects. Their figure of worship was the Divine Tree, a thousand-year-old cedar tree in Ebisugaoka.
After it was struck by lightning, the worshippers believed it had lots its power, leading them to worship the next oldest object in Ebisugaoka. This led them to the oldest Inari Shrine, which led to a fusion of the Tsukumogami Faith and Inari Faith.
However, as both faiths have different Gods, the Sennensugi Shrine's abandoned Tsukumogami to worship the Fox God. This led to their feud, where Tsukumogami wants to seek revenge on the Inari, now wanting to stop the wedding for the Fox God to not take hold of Hinako's powers, a rare child blessed by divinity.
