The following items are required to unlock the Sacred Sword in New Game+. These items are used to offer to the Jizo statues found throughout the game.
To find these items, you'll need to get clues that drop hints to their locations, and are mostly relevant during the chapter the statue appears in. However, you can backtrack to these locations to collect and offer their required items.

The first Jizo Statue can be found in Chapter 1 at the part where you're going to the Chizuruya General Store. Along the road, you'll be able to spot a red mailbox with the Jizo statue nearby. You can find the first clue for the offering inside the house southeast of the first Hokora.
The offering is located in the same house as the clue. You can find the offering inside a box in the storage room on the first floor. You'll need to find the Treasure Hunting Game Key in the house to unlock it.

The 2nd Jizo Statue can be found in Ebisugaoka Fields (Fog Town) as you make your way to Rinko's house. You can find it northwest of the fields near a tree and behind a hut. The clue for the offering is found on a wooden box outside a house with a hay roof.
For the offering, it can be found in the Sennensugi Shrine found directly north of the Jizo Statue. You can find the Rusted Flask on top of a rock surrounded by mini statues of foxes, right of where you're facing the shrine.
If you got the Sacred Sword already at least once during previous playthroughs, you can already obtain it at the 2nd Jizo Statue without collecting and offering the other items!

The 3rd Jizo statue can be found east of the Ebisugaoka Middle School. You can find it beside one of the houses at the edge of the mountain. The clue on the offering can be found on top of a large steel barrel in Ebisugaoka's streets.
You can find the offering for this statue across a broken bridge nearby. Just pass through the houses at the center towards the Ebisugaoka Middle School and head south to the broken bridge to get the Broken Japanese Geta Sandals.

The 4th Jizo Statue can be found during Chapter 8 - Ebisugaoka Mountain Path. You can find it on the roadside just a little bit further than the Hokora, south of Shu's house.
The clue for the 4th offering can be found in the Forgotten Shrine to the right of the platform with multiple stone hokoras. For the Cracked Hibachi Brazier, it can be found behind Shu's house, beside a shack.

The last Jizo Statue can be found east of Shu's house at the mountain during Chapter 10, behind a house. The clue for the offering can be found on top of a workbench outside the house, close to Shu's home.
The last offering, Dad's Old Kitchen Knife, can be found nearby, just east of the statue, stuck on a tree.
How to Use Dad's Old Kitchen Knife

Once all 5 Jizo Statues have received their offerings, head to the Divine Tree just west of the Ebisugaoka Middle School. When you approach the tree, a cutscene will play out, and you should have the Sacred Sword in your inventory right after.

If you want to get either the Bad Ending (Fox's Wedding) or the True Ending (Ebisugaoka in Silence), then you will want to purify the Sacred Sword. However, do note that purifying your Sacred Sword will have some notable disadvantages for the weapon.
The major drawback is that your Sacred Sword will no longer be able to regenerate its durability, unlike when it was unpurified. This means you'll have to use the Tool Kits to use it again.

If you don't plan on purifying your Sword, then you'll eventually lead yourself to the good ending of the game. It's also recommended to keep this sword unpurified if you're planning to get the remaining endings, as you will retain its regenerating effects.

To be able to purify the Sacred Sword, you must first solve the puzzle at the Hermitage of Crimson Water in the Dark Shrine Main Hall. You'll have to ring three bells around the area in the correct order to solve it.
First, ring the bell with the volcano symbol inside the temple. Then the bell with the waves symbol near the stone clue. Lastly, ring the bell with the tree symbol at the center of the area. It's important to know that you have to ring each bell only after the clacking sound.

To purify the Sacred Sword, you'll need to go to a small shrine located just northeast of Shu's house. From there, you should be able to see a tub of blood.
Simply go near it and interact with it to start the purification process. Note that once you've purified your Sacred Sword, it won't be able to give you protection from attacks and will not regenerate durability anymore.
While the Sacred Sword cannot use Focus Attacks, you will retain your Focus Gauge if you attack while in the Focus state.
If an enemy attacks you during this state, the sword will automatically block it for you, but you will be taken out of Focus.
The Sacred Sword can regenerate its durability when it is sheathed if you use other weapons to attack enemies. This way, you won't have to use that many Tool Kits anymore to repair your equipment.
