The Asakura Locker is the only required locker you need to open to progress the story. It is located inside the Middle School's locker room. For each puzzle difficulty, there is a different word mentioned in Asakura's Textbook.

The Aoi Takeshi Locker Code is based on the girl code cipher contained in the Origami of Grievances #2, which is located in the Old Annex. Using the girl code cipher, the code 401 translates to AOI in capital letters.
Entering the code unlocks Aoi Takeshi’s locker, where you’ll find Origami of Grievances #3 containing a clue to the Suga locker code.

The Suga Locker Code is mentioned in Origami of Grievances #3. Using the puzzle’s cipher, you can translate the SOS into digits. Here, the letter S looks like a 5, and O resembles a 0, giving you the code 505.
You can get a School Bag, which increases your inventory capacity by 1, after solving the Suga Locker Code!

The Taiko Locker Code belongs to Tsuchiya Taiko, as she is the only student with those initials. The clue from Origami of Grievances #1 is the most straightforward one, giving you the code 377 outright, as long as you know who it's for. Opening it will give you an Omamori Wolf.

All middle school locker codes are based on the Girl Code cipher, explained in the Note From a Male Student. According to the note, each number corresponds to a letter it visually resembles.
For example, the word SATOU in capital letters can be translated into the code 54+06, with S resembling a 5, A resembling a 4, and so on. This same logic applies to all the clues written on the Origami of Grievances, which are used to unlock the optional lockers in the room.

In the middle school, there are two Notes Between Lovers that use the same Girl Code. These are supplementary clues to the puzzle, but can be ignored since they don't give clues to the actual locker code combinations.
Note Between Lovers #1 Location

All the locker code puzzles can be found inside the Middle School, on the first floor. It requires you to unlock the locker room with the Locker Room Key found in the Old Annex.
Do note that if you're playing on New Game+, the locker room will automatically be unlocked, so there's no need to retrieve the key.

You can get the Key to the Back Mountains after entering the Asakura Locker Code. This lets you progress the story, as Hinako and her friends will be setting out once again.

Upon opening the optional middle school lockers in your first playthrough, you will receive an Omamori Wolf and a School Bag after opening Suga and TT's lockers, respectively. Meanwhile, subsequent New Game Plus runs will give you a Shriveled Abura-age and Dried Carcass.
