While progressing through Chapter 5 in your New Game Plus, offer the Broken Japanese Geta Sandal to the Jizo Statue located along the mountain path between Ebisugaoka and the Middle School. The statue can be found tucked away in a corner just outside one of the nearby houses.

During your New Game Plus Chapter 5, before you reach the mountain behind Ebisugaoka Middle School, go to the hidden path by the school entrance.

At the end of that path are some wooden planks and a box containing the Broken Japanese Geta Sandal.
