Completing 100% of Luigi's Mansion 2 HD (Dark Moon) involves playing both the Dark Moon Quest Mode and the ScareScraper Mode, as well as gathering all of the collectibles. Check out a full checklist of the things you need to do to 100% the game.

The most obvious step to reaching 100% completion is to finish all 34 missions in the Dark Moon Quest Mode. This involves going through all five mansions and defeating King Boo. Finishing all missions should reward you with one E. Gadd medal.

It's not enough to just finish all missions to 100% the game. You need to do well on each mission and earn 3 stars for all of them in order to get another E. Gadd medal.
Ranking System Explained: How to Get 3-Stars
You can upgrade your equipment by depositing the required amount of treasure into the vault. To get all of the upgrades available, you'll need to collect at least 20,000G in total.
Each mansion has 13 gems hidden within them that you can find while going through the missions. Finding all 13 gems in a mansion will reward you with a Luigi statue, for a total of 5 Luigi statues that you can earn.

Each regular mission contains one hidden Boo that you can find using your Dark-Light Device. Collecting all Boos in a mansion unlocks a bonus mission, which you also need to complete in order to 100% the game.
Boo Locations: Where to Find All Boos
Fill your Ghost Container at the vault by catching at least one of every type of ghost. It should be easy enough to catch one of every ghost in Evershade Valley by playing through all of the missions. However, there are also ghosts that are exclusive to the ScareScraper Mode.
The ScareScraper ghosts are a bit trickier to collect, since the boss ghosts in ScareScrapers are random. You will need to play through several ScareScrapers in order to find all of the different ghosts.
If playing with other people, the process can be faster by playing through Rush Mode. Take note that if any of your allies catches a ghost, it will be recorded in your vault as well, so there's no need to fight over who catches the ghost.
Hunter Mode may be easier to get through for those playing solo. Don't forget to play through Polterpup Mode, since the Polterpup ghosts are exclusive to this game mode.
Not all ScareScraper modes are available by default. Surprise Mode can be unlocked after completing ScareScrapers in each mode—Hunter Mode, Rush Mode, and Polterpup Mode. Endless Mode can be unlocked after completing a 25-floor ScareScraper.
Multiplayer Guide: How to Play ScareScraper
Once you've completed 100% of the game, three E. Gadd medals should be displayed on top of your save file. Each medal can be obtained by completing certain sections of the game.
One medal can be obtained by completing the all Dark Moon Quest missions. Another medal is obtained by getting a 3-star rank on each of the missions. The third medal can be obtained by completing the vault, which includes finding all of the collectibles and getting all of the equipment upgrades.
