Choosing to simulate a Witcher 2 save file will give you additional dialogues that changes how the story will play out. This will first be encountered in Vizima during the quest called Imperial Audience, where Voorhis will start to interrogate you during the beard shaving scene. You will be asked with a series of questions where some will have an impact on specific Side Quests.
Killing or sparing Aryan La Valette will only have a small impact in the game and it can only be seen on the quest called Broken Flowers where the dialogues inside the Valettes villa will slightly change.
Choosing a side will not have any significant impact on any Side Quest. However, how the story will playout and dialogues during the interrogation will slightly change.
The side you picked on the previous question will have a small impact on the dialogues available for this question. You can only choose the dialogue of save Anais if you chose to side with Roche on the previous question. While save Saskia dialogue is only available if you sided with Iorveth. The save Triss dialogue is available regardless of who you sided with.
If you choose to save Sile de Transarville from the malfunctioning megascope, you will encounter her inside a cell during the quest called The Great Escape. But if you choose not to save her, she will not appear in the game.
The last question will ask you if you spared or killed a fellow witcher named Letho of Gulet. If you answered that you spared him, there will be a follow up question asking where he is hiding. Choosing to spare Letho will give you the secondary quest called Ghost of the Past.

If you want a similar game, try playing Kingdom Come Deliverance, it's first person and not as smooth but it gives me the same vibes as the Witcher but a lot more realistic and hardcore
