You can only get the Magic Acorn during the quest Bald Mountain. You can loot this item on Imlerith's body after defeating him at the peak of Bald Mountain.
You must say No to Ciri when she asks you if you want to leave the mountain. If you say Yes, you will never be able to grab it again because the mountain entrance will be permanently closed.
Eating the Magic Acorn will give you 2 Ability Points that you can use to learn skills in the character menu. This makes the Magic Acorn one of the most valuable single-use Items in the game.
You can choose to not eat it, and to instead give the Magic Acorn to the villagers living below the peak of the mountain, but it will not have any impact on the story. As such, you can feel free to use it for its effects without any fear of affecting the story.
Talking to Stregomir by the fire at the beginning of the quest will provide you the history of the Magic Acorn and how it is used to cure illnesses for people of Velen.
