It’s recommended that you use the Red Capsules during your first playthrough of Silent Hill f as you'll get the Coming Home to Roost ending no matter what.
Red Capsules are very helpful for healing and it increases your maximum sanity.
Coming Home to Roost (Normal Ending)

If you are on your second Silent Hill f playthrough, note that some endings require you to avoid Red Capsules. These endings are The Fox's Wedding, The Fox Wets Its Tail, and Ebisugaoka in Silence.
You can also double-check the endings tab in the main menu to see if you can use Red Capsules or not!
Ending Guides and Explanations
If you did not take any Red Capsules during a run, you will obtain the trophy called Winners Don't Do Drugs. Do note that this trophy is unobtainable during your first run since you are bound to get Coming Home to Roost.

You can use Red Capsules as early as Chapter 1 to restore health or increase sanity. Be sure to assign them to an item shortcut in the game's inventory while exploring or fighting enemies for easy access.

The first time you'll get Red Capsules and the Capsule Case will be after escaping the Fog Monster in Chapter 1 – Ebisugaoka. After that, you can naturally find them while progressing through the story as they are scattered around Ebisugaoka.
Keep in mind that the locations mentioned are just examples and may vary depending on the game’s difficulty. Don’t hesitate to check every corner—you might find more Red Capsules hidden away!
