To begin this sidequest, you need to talk to Dave. Go to Meldacio Hunter HQ, and look for Dave on the balcony of a house. Speak with him to start a conversation about finding the Dog Tag. Finish the conversation to start the sidequest.
Open your map, and choose the Mountains of Misfortune sidequest. Follow the location shown to find the Polished Dog Tag. Take note that Cockatrices are protecting the area, defeat those first before looking for the Polished Dog Tag. You'll see the Dog Tag near the small ruins.
Go back to Meldacio Hunter HQ, where you found Dave. Then, give the Polished Dog Tag to complete this sidequest.
