Broken Vinyl Record (Other Half) and Vinyl Glue | Groovy Record Store |
Use the Vinyl Glue to attach the two Broken Vinyl Halves together and create a Glued Up Vinyl Record. Head back to the Jukebox in Neely's Bar to attach the missing Jukebox Button, and insert the Coin to solve the puzzle.
The Grand Market Keypad code can be found on a Code Note, but you can enter the correct code directly, based on your game's puzzle difficulty, without needing to search for the note. Unlocking this is purely optional but will reward you with supplies!

Collect the Coin (Man) from the Room 206 Safe, the Coin (Woman) from the Courtyard after unblocking the garbage chute, and the Coin (Snake) from the Pool Area. Afterward, arrange them on the Coin Cabinet corresponding to the scenarios shown on the top of the cabinet. The riddles will change depending on your game's difficulty, but can be arranged by following the diagram above.

The Apartment Safe Code is tied to the poem and drawings on the wall, with the code varying depending on the difficulty of the puzzle:

Find and set the Hour Hand to 9, the Minute Hand to 2, and the Second Hand to 3 immediately after obtaining each of them, as setting the hands to their correct positions will open up new doors within Blue Creek Apartments.

Find the Pigeon Figurine in Room 210, then combine the Wooden Swan Head from Room 211 and the Malformed Figurine Part from Room 209 to form a Swan Figurine. Once collected, Place both figurines on opposite sides of the seesaw until the bar is balanced evenly to solve the puzzle.

The Moth Combination Lock is derived from the symbols on moths within the room used to answer the equation by the door. You may choose to skip doing those steps and enter these codes depending on your game's puzzle difficulty:
The Jacks Inn Motel Safe Code can be found on a Toolbox Memo in Room 107 but even without finding the note, you may input these codes based on your game's puzzle difficulty:
Motel Safe Code Guide (Jacks Inn)
The Bowling Alley Safe code is 1887 for all difficulties, and can be used without finding the note that contains the code. This safe is only accessible in New Game Plus and will not appear in your initial playthrough even though the Bowling Alley is accessible at that time.
The Hospital Padlock Code takes its answers from the Photo of Nurses, Photo of Doctors, and Photo of Trees around the hospital's first floor, which will result in any of these codes depending on your playthrough's puzzle difficulty:

Nurses Station Keypad Code Guide
The 3rd Floor Hospital Door code across all difficulties is 4-37-12 and can be unlocked by turning the knob in the direction of the arrows written on the door.

After combining the Moldy Radiograph with the Mold Remover to create a Cleaned Up Radiograph, place it along with the Radiograph of a Pelvis on the X-ray viewer in Exam Room 4 on the third floor. Align the scratch marks on the corners of all Radiographs to solve the puzzle and reveal the code: 4-37-12.
Solving this puzzle is optional to unlocking the 3rd floor hospital door once you already know the code.

Collect the Marked Bracelet in the Medical Records room, the Bloodstained Bracelet from the Pool, and the Filthy Bracelet from Room D1. Place them all on the Hand Puzzle's wrist, then align the numbers on their clear parts to get the numbers: 92-45-71 which solves the puzzle.
Arrange the Director's Books in the Director's Storage Room until they form an image of a fire-breathing lion, then assign the numbers 1-9 on the symbols written on the spine as a reference to the Hospital Safe Code.
The Hospital Safe Code will require the number combination: 92-45-71 from the Hand Puzzle, and the symbols from the Director's Books. Match the symbols to the number combination and input the proper sequence on the Hospital Safe to unlock it.
The answers for the Elevator Quiz is 2-3-1. Knowing the answers to the quiz is optional and will not halt your game progress.
Hospital Elevator Quiz Answers Guide
Take the Elevator Quiz Answers: 2-3-1 and input it on the Q1, Q2, and Q3 rows of the box from top to bottom to open the box.
Reveal the Keypad Code: 9659 from the Room 17 treatment stretcher, piece together the Combination Lock Code: 1622 from Room M3 and M1, and retrieve the Lapis Eye Key from the Surgical Chair training dummy in Exam Room 4. Use all of these components to unlock the Chained Box and progress the area.

The code in the Bug Room will be in a random arrangement each playthrough regardless of puzzle diffculty, though will always contain the numbers 2, 3, and 9. You can try a myriad of combinations listed above, on the keypad.

Each time you collect a new weight, return to the scales and place them on the pans to unlock the corresponding door. Behind each door is another weight to collect, with the ultimate goal being to gather all the weights and use them to finally unlock the Hangman Puzzle.

Flip all of the switches on the Generator in sequential order to overload the power and solve the puzzle. The number of switches and its sequence will vary depending on your game's puzzle difficulty.
The answer to the Hangman Puzzle can be any noose through trial and error, though you may match all of the torn verses to each other by visually matching them based on the tear lines, then make your judgement on who the innocent criminal is.

Turn the Rotating Cube’s face to unlock different sections of the Labyrinth. The correct symbols are consistent across all difficulties, and reaching it correctly will grant you access to new areas where additional challenges await.

Once you have collected the Snow White Figurine from Room 201, the Cinderella Figurine from the Conference Room, and The Little Mermaid Figurine from Venus Tears Bar, place them on the music box and align the disks to create a path to their designated door. To open the doors, you must turn the keys a certain number of times from left to right depending on your game's difficulty:
Find an extra Mirror Shard in Room 202's bedroom, and piece together all of the mirror shards on the frame. You can place either the Rotten Apple or Ripe Apple on the platter in front of the mirror, as either will solve the puzzle.

Match the symbols engraved on the book spines to their corresponding positions shown in the painting to correctly align the books and solve the puzzle.
Finding Suitcase Code will need you to repair the vanity first with a Red Light Bulb, although you may choose to skip that step and instead apply any of these codes depending on your game's difficulty:

The Hotel Safe Code is always 7414 regardless of your puzzle difficulty which can be found by solving the optional Gem Box puzzle in the Manager's Office.
Puzzles are major mechanics for progressing in Silent Hill 2. They are mostly environmental puzzles that allow you to access otherwise locked or hidden areas and progress the story!
