Take Care of James and Remember Mary

To get the Leave Ending, you need to make sure that James is well taken care of by keeping his health up immediately at all times, to symbolize his will to continue living.

You must also allow him to keep the memory of Mary close to his heart by examining Mary's photo and letter in his inventory, and spending as little time as possible with Maria to reinforce James' emotional connection with his late wife.

Always Keep James' Health Up

Make sure to always heal James as soon as possible using Syringes or Health Drinks whenever he sustains damage. If you are forced to have him in a critical condition, make sure to heal him once you get a healing item.

Sustaining James' health gives the impression that James cares about his own well-being, and that he wants to stay alive. This condition also steers the game away from In Water Ending, which needs James at low HP all the time.

Examine Mary's Photo More Than Once

Keep examining Mary's photo and letter in James' inventory when you can, at least three times in the playthrough. This contributes toward James' eagerness to search for his wife, and reminds him of his love for her.

Listen to Mary's Monologue

While walking down the last hallway in the Lakeview Hotel Otherworld before confronting the game's final boss, Mary's voice can be heard going on an angry tirade as she is faced with her own mortality.

Listen to the entire monologue until it stops with Mary softly apologizing, then proceed through the door.

Do Not Check on Maria

Since the Leave ending is about James confronting his guilt toward Mary, make sure that this is reflected by spending as little time as possible with Maria.

Ignore her advances while she travels with you to avoid building rapport with her, as that is a condition that can lead to the Maria Ending instead. Do not let her die to monsters, but don't go out of your way to protect her either.

Do Not Return to Rooms Where You Leave Maria

There are two instances where James can leave Maria behind. Specifically, he leaves her to rest in one of the rooms at Brookhaven Hospital, and during the part further in the story where she is found dead inside a cell in the Labyrinth.

In both instances, make sure you never go back to those rooms after exiting, as to avoid fulfilling conditions for the Maria Ending.

Do Not Inspect Angela's Knife

When Angela leaves her knife with you after meeting her again in Blue Creek Apartments, make sure that you never inspect it from your inventory more than once to steer James away from thoughts of self-harm.
