Room 312 |
To reach the UFO Ending, players have to finish at least one full playthrough of the game. Then, on a New Game Plus file, find the Blue Gem at the Jeweller's Store north of Big Jay's store on Neely Street. The Jeweller's Store can be found in front of the police car on the street.
Inspect the Blue Gem in all of the locations shown above, ending with Lakeview Hotel's Room 312. Successfully inspecting the gem in the all of the prior areas will cause the game to cut to the UFO Ending instantly when inspected in Room 312, with no final boss fight.

After completing your first playthrough, you can get the UFO ending by starting a New Game Plus save file and collecting the Blue Gem which will be the key item used to unlock it.
New Game Plus Guide: All Unlockables
You can find the Blue Gem placed on top of a jewelry box displayed on the Jeweller's Store next to Big Jay's in Eastern South Vale.
Smash through the window and collect the Blue Gem. Note that this item will only appear at this location after you have completed at least one playthrough of the game.
Blue Gem Location and How to Use
To reach the ending, you have to inspect the Blue Gem from your inventory in three locations: the rooftop of Saul Apartments at Eastern South Vale, the pier where you meet Maria at Rosewater Park in Western South Vale, and at the docks when you arrive at Lakeview Hotel.
Then, you will need to inspect it one last time in Room 312 of Lakeview Hotel. You will know that you are using the Blue Gem at the right place if you see it glow, accompanied by alien-like noises. If done correctly, using it in Room 312 will make the game cut to the UFO Ending.
You can find a magazine on a couch in the Groovy Music store on Neely Street that provides hints about the location of the Blue Gem, as well as each of the locations you have to use the Gem at.
The UFO Ending is one of two joke endings that were present in the original game, and have returned in the remake with some changes. In this game's version, James meets Space James, who is using his model from the original game.
Space James reveals that James is just a clone, before the latter gets shot with a ray gun by an alien, and subsequently abducted. This easter egg ending continues the long-running joke of Silent Hill games featuring UFO endings, a tradition seen in several mainline titles.
